Nursing Care Plan for Endocarditis - Assessment

Endocarditis is an infectious disease caused by microorganisms of the endocardium or heart valves. Infective endocarditis usually occurs in the heart that have been damaged. Activity / rest:
Subjective data: Fatigue, weakness

Circulation

Elimination
Subjective data: history of kidney disease or kidney failure, history intake decreased urine frequency.
Objective data: cloudy urine concentration.

Comfort:
Subjective Data: Chest pain in the anterior part (hard / sharp) as inspiration, cough, move, lie down; reduced pain when sitting, chest pain moved to the back.
objective data: Restless.

Respiration:
Subjective data: Shortness of breath, worse at night (myocarditis).
Objective data: nocturnal dyspnea, cough, wheezing Inspiration, tachypnea, crackles and Ronchi weak, slow respiration.

Security:
